The objective of the present work is to describe the clinical manifestations, diagnostic methods, therapeutic management as well as the intraoperative characteristics of the epidermoid cyst. The case of a 37-year-old male patient is reported, who presented with absence crisis, headache, diplopia, left hearing loss and limitation of ambulation of 6 months of evolution. A brain magnetic resonance imaging (MRI) study is performed in which a hypointense supra and infratentorial lesion is observed with a signal similar to that of cerebrospinal fluid (CSF), with little contrast uptake and hyperintense in T2 sequence. Surgical treatment with total resection was performed with a favorable postoperative evolution and no evidence of recurrence at 2 years.
